WHAT IS A COSMETIC ANTI WRINKLE TREATMENT USING BOTULINUM TOXIN TYPE A?

There are many commercial brands of Botulinum Toxin available in today’s market, all of which are nearly identical in composition and action. They include Nabota/Nuceiva/Juveau and Botulax from Korea, Botox from the USA, Xeomin/Bocouture from Germany and Dysport from the UK.

At Carleton Cosmetic we are fortunate to be able to provide and source the finest Korean products, direct from the manufacturers, despite not yet being generally available in New Zealand. Other brands can be used with prior notice request but will be subject to specialist order and higher cost.

The full effect of the Btx is achieved after 3  weeks preceding  treatment and then there is a gradual return of muscle movement  over the next 2–3-month period. There is no hard and fast rule for how quickly movement returns, since the regrowth of nerve endings is directly correlated to each individual patient’s metabolism and prevailing lifestyle.

Most patients renew their “normal” Botulax treatment at around 3 months and for those electing for “Nabota” they can expect at least a 50% increase in duration of action.

Frown lines between the eyes, (the glabella), is the most common area for anti-wrinkle treatments. Crow’s feet and forehead lines are the next most popular and Btx is also used to great effect in the lower face to correct gummy smiles, chin and marionette lines as well as prominent  Platysmal bands in the neck.

Btx takes 5-10 days to begin activation and is at its maximum effect after three weeks. It is normal to experience some asymmetry during the first 2 weeks, and any corrections are not carried out until the period after this.

In some cases, when someone is having their first treatment, we may intentionally slightly under treat and then review after two weeks. This is done to avoid over treating (mostly forehead and crow’s feet) as everyone responds slightly differently, and it is easy to add further product but cannot be “taken away”. Once the correct dose is established, it is recorded and used for subsequent treatments.

The Truth About Filler Longevity: Longevity, Migration, and Fixes

Hyaluronic acid (HA) fillers primarily migrate and persist rather than simply dissolving on a predictable 6 to 12-month timeline. While the body’s natural enzymes do break down HA, cross-linking in fillers makes them highly resistant. Consequently, un-dissolved gel deposits can remain trapped and shift under the skin for years.

The Science of “Dissolving” vs. Reality

  • Long-Term Persistence: MRI studies have revealed HA filler residue lingering in the skin 5, 10, or even 15 years after the initial injection. The gel does not magically disappear; it integrates into your soft tissue and continues to draw in water.
  • Natural Breakdown Limitations: Your body naturally produces an enzyme (hyaluronidase) that breaks down HA. However, this natural breakdown is most active in the first few weeks following an injection. The body does not maintain sustained, high levels of this enzyme long-term, as it would destroy your body’s native HA.

 

Understanding Filler Migration

  • How It Happens: Filler does not just sit completely still. Over time, factors like gravity, repetitive facial muscle movements (smiling, chewing, talking), and lymphatic spread cause the gel to disperse and settle into surrounding areas.
  • Overfilling: Migration is highly accelerated by repeatedly “topping up” the same area with too much product over time, eventually causing the tissue to lose its shape and the filler to spill out of the original injection plane.

 

How to Fix It

If you notice lumps, puffiness, or volume where it shouldn’t be (such as above the lip border or under the eyes), the filler needs to be actively corrected:

  • Synthetic Hyaluronidase: A trained practitioner can inject synthetic hyaluronidase, an enzyme that safely breaks down and dissolves the HA gel, allowing your body to naturally reabsorb it.
  • Multiple Sessions: Because migrated filler is often spread out into the tissue, achieving a complete correction may require multiple sessions of hyaluronidase.
